Once you complete each form, an email will be sent to our office letting us know it has been completed. Because the court sets deadlines on this information, once your case is filed we will need all four forms completed NO LATER than TWO days after your filing date.
If you are unsure of your creditors or would like to confirm your creditor history, we recommend that you obtain a copy of your credit reports. If you go to www.annualcreditreport.com you will be able to access a free credit report from all three credit bureaus (Equifax, Experian & Transunion.)
It is your responsibility to make sure all of your creditors, income and personal property information are included on each correct form and that you have provided the necessary information for each item, such as your creditors (name, address and amount owed). We are not responsible for any creditor or personal information that was not included because you did not comply with the above requirements.
If you don’t have a computer or internet access, or if you can’t complete the online forms for any reason, please contact us right away and we will provide you with a paper copy of the client worksheet.
